South Western Sydney Residents Can Learn How to Plan for Future Care

Community members in South Western Sydney will have several opportunities to learn about advance care planning through a new series of workshops delivered in September, October and November.
The sessions are being arranged by South Western Sydney Primary Health Network (SWSPHN), which has engaged Palliative Aged Care Consultancy Service (PACCS) to deliver the program. The initiative is supported through funding from the Australian Government’s PHN Program.
PACCS works across NSW to provide palliative care and complex pain management support, along with education and consultancy for residential aged care services.
Sessions available online and in person
People can choose from an online webinar or face-to-face workshops in Moss Vale and Bowral.

Turning personal wishes into a clear plan
The sessions will give participants practical guidance on thinking about the healthcare they may want in the future and recording those choices.
Trained facilitators will explain how advance care planning works and help participants consider the treatments and care that may matter to them. Participants can also receive assistance with documents such as an advance care directive or a Values and Preferences Statement, where appropriate.
The group format also provides an opportunity for people to discuss the topic in a supportive setting and learn from others.
The workshops aim to help participants:
- Feel more comfortable talking about future healthcare decisions
- Get individual assistance with documenting their preferences
- Better communicate their wishes to family members and healthcare providers
- Understand the options available for recording their healthcare choices
The program is funded by SWSPHN and delivered by PACCS to improve community access to information and practical support around advance care planning.
